 ASSOCIATE MOVIOLA REGISTRATION FORM

We do not currently have a facility for submitting this form on line.  Please copy it into a Word document and send it to us by email or by post to the address at the foot of the page.  It forms our agreement with you for supply of films and will be filed as an official record, to be accessed in the event of any dispute.  If you are using email please type your name in the space provided for signature.   This will indicate your acceptance on behalf of your group of the Conditions of Service attached to this Registration Form.   Please file carefully a copy of  both this form and the Conditions of Service for your future reference.

NAME OF THE ORGANISATION TO WHICH OUR INVOICE SHOULD BE MADE OUT:

This could be your name or your Village Hall or your Parish Council or your Group or your Venue name.

NAME AND ADDRESS OF THE VENUE WHERE THE FILMS WILL BE SHOWN:

This is required for all bookings and will be supplied to the Film Distributor.

CHOICE OF NORMAL SERVICE – GOLD or SILVER:

This is the service level which will NORMALLY apply to your bookings.  Please see Note 11 if you are registering for the SILVER service.
